Are you a qualified Social Worker seeking a rewarding hospital-based role with scope for autonomy and professional growth? Join our South Transfer of Care Team, covering Addenbrooke’s Hospital, other local surrounding hospitals and Community Rehabilitation Units. You will work with adults aged 18+ to support safe discharge, arrange care and support, and undertake the six-week post-discharge review to ensure ongoing needs are met and outcomes are sustained.

What will you be doing?
You will undertake Care Act Assessments for adults to facilitate from hospital or rehabilitation services to identify care and support needs, plan and arrange timely care provisions, to enable a safe discharge. You will also undertake safeguarding enquiries as part of your Social Worker role. The South Transfer of Care team also complete six-week reviews post-discharge, to review the effectiveness of support and adjust care provisions as required. You will work within a multidisciplinary team alongside therapists, nursing staff and providers to deliver person-centred care, communicating effectively with service users, families and professionals and documenting decisions and outcomes clearly.
The Transfer of Care Teams operate within set office hours Monday to Friday and we will consider flexible working arrangements.
Our team provides strong peer and managerial support, regular supervision, and opportunities for professional development. Interviews are usually held face to face and may include an assessment but we’ll give you all of the relevant information ahead of time.
About you
We are looking for confident and dynamic Social Workers who enjoy making a difference in their local community.
You must be a qualified Social Worker with current Social Work England registration and be able to travel independently across the county and work from multiple locations.
You should be comfortable managing a busy caseload while prioritising safety and positive outcomes.
