About the role
An exciting opportunity has arisen within Prevention and Early Intervention Services for an Adult Support Coordinator based in the Adult Early Help Team.
Adult Support Coordinators play a vital role in supporting adults and older people who may require care and support, as well as their informal carers. You will work closely with individuals, families, and partner organisations to understand people's strengths, identify what matters most to them, and connect them with the right services and support to maintain their independence and wellbeing.
The Adult Early Help Team is the front door to Adult Social Care, providing advice, guidance, information, and onward referrals to people across Cambridgeshire. Our focus is on prevention and early intervention, helping people find solutions at the earliest opportunity and reducing the need for more intensive support wherever possible.
This is an ideal role for someone who enjoys problem-solving, building relationships, and making a positive difference through conversation and timely intervention. Rather than undertaking long-term case management, you will support people to access community resources, preventative services, technology-enabled care, voluntary sector provision, and other sources of support that can help them remain safe, well, and independent.
Our multidisciplinary team works collaboratively to support individuals, families, and carers. We work closely with colleagues across Adult Social Care, Health, Housing, and the voluntary and community sector, sharing knowledge and expertise to ensure people receive the right support at the right time.
You do not need to hold a social work qualification to apply. However, knowledge, experience, and understanding of the needs of vulnerable adults will be essential. Experience within health, social care, housing, customer services, welfare support, or a related setting would be advantageous. A working knowledge of the Care Act, Mental Capacity Act, and Adult Social Care policies and procedures would also be beneficial, particularly in relation to wellbeing, strengths-based practice, and person-centred outcomes.

What will you be doing?
As an Adult Support Coordinator, you will often be the first point of contact for people seeking support from Adult Social Care. Through telephone and video conversations, you will work with adults, older people, and their carers to understand what matters to them, identify their strengths and support networks, and explore solutions that help them remain independent.
You will complete Community Action Plans and Carer Conversations, supporting people to achieve their personal outcomes through information, advice, guidance, signposting, and referrals to appropriate services. Using a strengths-based approach, you will work creatively and collaboratively with individuals to identify opportunities within their local communities, helping them access support that prevents, reduces, or delays the need for ongoing Adult Social Care involvement.

You will use your curiosity, problem-solving skills, and professional judgement to explore a wide range of circumstances, working alongside colleagues and partner organisations to find personalised solutions. Whether connecting someone to community services, supporting a carer to access help in their own right, or identifying preventative interventions that improve someone's wellbeing, your work will make a real difference to people's lives.

About you

You will be an excellent communicator, able to build rapport and trust quickly through telephone and video conversations. You will have strong listening skills, be naturally curious, and feel confident having meaningful discussions that help people identify their strengths, needs, and goals.
You will be organised and able to manage a varied workload, using sound judgement to prioritise tasks and respond effectively to changing demands. The role offers a high degree of autonomy, so you will need to be comfortable working independently while also recognising when to seek support, advice, or collaboration from colleagues and partner organisations.
We are looking for someone who is resilient, adaptable, and enjoys problem-solving. You will be able to analyse information, think creatively, and work with individuals, families, carers, and professionals to identify practical solutions that promote independence and wellbeing.
Good literacy, numeracy, digital, and administrative skills are essential, alongside the ability to maintain accurate records and use information effectively to support decision-making.
You will have a good understanding of the Care Act 2014, the Mental Capacity Act 2005, and the principles of person-centred and strengths-based practice. Experience of working alongside a range of professionals and organisations to achieve positive outcomes for individuals will be advantageous.
