Adult Support Coordinator - City and South Community Team

We’ve changed the way that we structure our Adult Social Care teams in Cambridgeshire and have an opportunity for you to join our Adults Community Teams as an Adult Support Coordinator.

We have a vacancy in our Cambridge and South Cambridgeshire Team, based in Cambridge.
 
We’re embracing new ways of working so our social work teams can work where they need to and have the chance to be part of a collaborative team environment with strong working relationships with health professionals and partners to achieve better outcomes for the adults we support.

What will you be doing?

You’ll be liaising with other departments, undertaking Social Care and Mental Capacity Act assessments, as well as care and support planning to achieve better outcomes for the adults we support.

You will have a genuine desire to support people and you’ll put the individual at the heart of everything. You'll need to be inquisitive and provide evidence based, proportionate recording with a keen focus on strength-based practice.
