Adult Support Coordinator - Community Team

We have restructured our Adult Social Care teams in Cambridgeshire and invite you to join our Adults Community Teams as an Adult Support Coordinator. We have a vacancy in our Cambridge and South Cambridgeshire Team, based in Cambridge.

We embrace new ways of working, allowing our social work teams to work where they need to. You’ll be part of a collaborative team environment with strong working relationships with health professionals and partners to achieve better outcomes for the adults we support.

Interviews will be held by Microsoft Teams and will take place the week beginning the 10th of February.

What will you be doing?

You will liaise with other departments, undertake Social Care and Mental Capacity Act assessments, and plan care and support to achieve better outcomes for the adults we support.

You must have a genuine desire to support people and put the individual at the heart of everything. You need to be inquisitive and provide evidence-based, proportionate recording with a keen focus on strength-based practice.
