Adult Support Coordinator - South Transfer of Care Team - Fixed-Term for 1 year

About the role

Are you an experienced Adult Support Co‑ordinator looking for a rewarding hospital‑based role with scope for autonomy and professional development? Join our South Transfer of Care Team, working across Addenbrooke’s Hospital, surrounding local hospitals, and Community Rehabilitation Units. You will support adults aged 18+ to plan safe and timely discharges, arrange appropriate care and support, and complete the six‑week post‑discharge review to ensure ongoing needs are met and positive outcomes are sustained.

The Transfer of Care Teams operate within office hours, Monday to Friday, and we are open to discussing flexible working arrangements. Our team offers strong peer and managerial support, regular supervision, and opportunities for ongoing development. Interviews are usually held face to face and may include an assessment, and we will provide all relevant information in advance.

This is a fixed-term role, for a 12 month period.

What will you be doing?

You will carry out Care Act Assessments for adults preparing to leave hospital or rehabilitation settings, identifying their care and support needs and arranging timely provision to enable a safe and well‑planned discharge. You will also contribute to safeguarding enquiries as part of your responsibilities within the Adult Support Co‑ordinator role.

The South Transfer of Care Team completes six‑week post‑discharge reviews to evaluate the effectiveness of support arrangements and make any necessary adjustments to ensure outcomes are sustained.

Working as part of a multidisciplinary team, you will collaborate closely with therapists, nursing staff, and care providers to deliver person‑centred support. Clear communication with adults, families, and professionals, along with accurate recording of decisions and outcomes, is essential.

About you

We are looking for confident and proactive Adult Support Co‑ordinators who are passionate about making a positive difference in their local community. 

You should have experience working with adults in a health or social care setting and be able to travel independently across the county, working from multiple locations as required. 

You will need to be comfortable managing a busy and varied caseload while maintaining a strong focus on safety, wellbeing, and positive outcomes.
