Adult Support Co-ordinator with the Living in Care Home Review Team

Please Note: The application deadline for this job has now passed.

About the role

Do you enjoy making a difference to your local community?  Are you a curious practitioner who wants to make a difference to the lives of the individuals who we support?

If the answer is yes, then we have an opportunity for you to join our Living in Care Home Review Team.

We are looking for an Adult Support Coordinator to join on a full time, permanent basis.

We are a Countywide Team responsible for residential and nursing care customer reviews in and out of the county and completing social care assessments for customers entering adult social care services, giving you the ability to develop working relationships with customers, their families, our providers and our health and social care partner organisations.

Join us and be part of an authority that has accepted the challenge to constantly improve.

If you would like an informal chat about this role please contact Sara Culbert, Living in Care Home Review Team Manager, on 07771 975073.

We are looking for exceptional Social Workers to join us on our journey. For an informal discussion about our vacancies, contact Beth on 01480 372 425 or e-mail welovesocialworkers@cambridgeshire.gov.uk.

We are planning to hold interviews for this role in the week commencing the 10th of June.

We know that your time is precious, so our interviews are generally held virtually, by Microsoft Teams, to provide the most flexible recruitment process. We may include a virtual assessment as part of this, but we’ll give you all the relevant information ahead of time.

Appointment to this post will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service. The Council is committed to safeguarding and promoting the welfare of children and vulnerable adults and expects all staff and volunteers to share this commitment.

What will you be doing?

You'll need to be inquisitive and willing to spend time finding out what it's like from your customers point of view to live in the setting that they do and build knowledge to improve the quality of living for your customers. You'll need to have excellent observation skills to spot strengths and notice what's lacking and let the facts drive your ability to give constructive feedback and provide evidence based, proportionate recording. Your curiosity should also mean that you enjoy keeping up to date with current good practice, doing relevant research and building your own knowledge.

This is not your typical team - there will be an opportunity to broaden your own skills and training by working alongside nurses and learning about medical practices and you will support providers and share your knowledge and experience. Completing Social Care Assessment, Mental Capacity Act Assessments, Continuing Health Care checklists and attending Decision Support Tool meetings, completing Safeguarding Enquires and engaging with Advocacy Services will be part of the role but you do not need to be an expert, full training and support will be provided.

There will be an occasional need to support working out-side of core working hours (early morning / late evening) and support over weekends. This is only occasional due to potential provider failure or emergency situations.

About you

Experience or a willingness to work with a variety of partner organisations is vital to working in this team and you'll need to enjoy networking and influencing others to see things differently. You will be able to work autonomously and have the flexibility to manage your own workload in a hybrid way that allows you to work from home, an office base and the residential settings in your patch team area.

Most of all, you put the individual at the heart of everything and you ensure their choices are being respected and supported.

To apply, you must have:

  •  Level 3 qualification i.e. A level’s grade A-E, NVQ or equivalent and experience of    working in Adult Social Care Or Level 4 Degree level qualification or  equivalent.
  • Have the ability to work from a variety of locations and travel countywide independently.
