About the role
At Cambridgeshire County Council, we are dedicated to making a positive difference within our community. We believe in the power of early intervention and prevention to transform lives, and as we expand our efforts to reach even more individuals and communities, we are seeking a self-motivated and energetic Prevention & Early Intervention Care Coordinator to join our Reablement team and lead the way in shaping the future of our services.
So, are you a dynamic and driven professional looking to make a real impact within the community? Can you help support the delivery of high-quality care and support services that aim to promote and sustain people’s independence? If so, we have a fantastic opportunity for you within the Prevention & Early Intervention Service for Cambridgeshire County Council.
The role involves working alongside Reablement Service and Reablement Duty Coordinators to ensure quality standards are met within work practises, and guidance and support is given to support staff to enable safe and effective service delivery.
You will work with wider preventative services and teams to ensure that service delivery throughout the reablement pathway is well coordinated to ensure that the reablement service achieves the best quality outcomes for service users, their carers and families.
The post holder will be responsible for ensuring that they work within organisational policies, procedures, and guidelines and comply with Care Quality Commission regulated standards, and you will follow guidelines including but not limited to Data Protection, confidentiality and information sharing protocols, and multi-agency safeguarding procedures.
For more information, or to chat about this opportunity, please contact Angela Smith at angela.smith@cambridgeshire.gov.uk
Interviews will be held on Wednesday, 18 October 2023.
What will you be doing?
Working together with Reablement Coordinators, the position requires working primarily between Monday and Friday; however, you may be asked to contribute toward weekend and evening working to support the Reablement team.
About you
You will need to be organised, proactive in your approach to demand, and be able to work autonomously using your own initiative, whilst prioritising your own workloads. You will also be required to support Reablement Services across the County, and so will have the means and ability to travel.
