Senior Practitioner - Prevention and Early Intervention

About the role

An exciting opportunity has arisen for a new role within Prevention and Early Intervention Services for a Senior Practitioner.

The role will require you to be a qualified and registered Social Worker or Occupational Therapist.

The role will be responsible for the clinical oversight and management of referrals into the Prevention and Early Intervention Services via the Prevention Hub, including the adult early help, reablement and front of house social work teams. The Prevention Hub ensures the most appropriate service is provided, in line with criteria, to meet the needs of the people who require early intervention, supported by short term care support, including appropriate equipment and assistive technology to improve and recover independence. 

You will work closely with external colleagues to ensure that the ethos and pathways of the prevention services are understood and service users needs are met with the most appropriate service. Supporting social workers and liaison coordinators within the front of house hospitals.

As a council Cambridgeshire will support individuals to look at their individual learning outcomes and offer a wide range of training and apprenticeships.

What will you be doing?

You will work collaboratively with colleagues across the health and social care sectors to develop procedures, processes and systems.  You will manage staff to ensure effective and efficient service delivery, ensuring compliance with Care Quality Commission (CQC) regulations, Care Act and Cambridgeshire County Council (CCC), policies and procedures. 

You will be responsible for ensuring that new referrals in the service are processed in a timely manner to the most appropriate service within prevention and early intervention. You will be line managing and supporting staff across the county so will need to have the ability to travel.

About you

You will need to have good problem solving skills, management of KPIs, good multidisciplinary working skills and experience of line management. You will need to have good and accurate literacy, numeracy and administrative skills and the ability to analyse information to ensure best approach. It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative. Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience.

You must be a qualified Social Worker or Occupational Therapist with current valid registration
