Social Worker - Prevention and Early Intervention Assessment Team

About the role

An exciting, full time, permanent opportunity has arisen within the fast paced, Countywide Assessment Team for a qualified Social Worker.

The Assessment Team complete Care Act Assessments and Care and Support Plans for anyone over the age of 18, who is seeking support or social care following a period of Reablement. They also support unpaid carers through assessment and support planning.

As part of the wider Prevention and Early Intervention Team, we work closely to assist colleagues within the service to deliver the best outcomes for our service users. Our Social Workers mentor Reablement leads and are crucial to the duty function within the team.

The team is made up of Social Workers and Adult Support Coordinators from a variety of backgrounds. The team offer support to individuals and also to families and carers. The team work closely to support and share information on prevention services across the county.

As a council, Cambridgeshire will support individuals to look at their individual learning outcomes and offer a wide range of training and apprenticeships.

What will you be doing?

You'll be working with individuals and their informal carers to complete Care Act assessments, care and support plans, community action plans and carers assessments, alongside having a role in Safeguarding and considering capacity.

Inquisitive and creative practice is embraced, to allow for personalised solutions, taking into account wider community resources and organisations. The work will be ensuring early and safe discharge from Preventative services, working closely with social care and health colleagues to ensure correct discharge plans are in place.

About you

You must be a Qualified Social Worker with current Social Work England Registration

You will need a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Experience of multi-disciplinary working to enable best outcomes for individuals is key.

It is imperative that you have strong communication skills, a confident telephone manner and can work on your own initiative.

Good planning and organisation skills will be key, and you should be used to working under pressure to meet deadlines, allowing flexibility and resilience.
