Adult Support Coordinator - Learning Disability Partnership

About the role

The Fenland Learning Disability Team have an exciting opportunity for a fixed term adult support coordinator post for 12 months. The posts is for 37 hours per week and the office base is in Hereward Hall in March. A secondment opportunity would be considered for internal applicants.

Adult support co-ordinators play a major part of the work we do in our team. You’ll be working with adults with a Learning Disability with care and support needs, along with their informal or formal carers.

What will you be doing?

You'll be working with individuals to complete adult social care assessments, care and support plans, mental capacity assessments and carrying out carers conversations as well as other team tasks and responsibilities.. You will work as part of an MDT with both health and social care colleagues.

About you

You won’t need to have a social work qualification but knowledge, experience and understanding of the needs of vulnerable adults will be essential to succeed in the role. Working knowledge and experience of the Care Act, Mental Capacity Act and Cambridgeshire County Council's Adult Social Care policies and procedures will also be an advantage.

Good planning and organisation skills will be key. You will need to have good written and verbal communication skills and the ability to communicate with adults with a variety of needs and communication styles.

Inquisitive and creative practice is embraced, to allow for personalised solutions, taking into account wider community resources and organisations.
