The South Transfer of Care Team is seeking a full‑time Senior Social Worker, based primarily at our Trumpington office near Addenbrooke’s Hospital in Cambridge. While this is a countywide service, the South team mainly supports discharges from Addenbrooke’s hospital, as well as Papworth Hospital, West Suffolk Hospital in Bury St Edmunds, Newmarket Hospital (Rosemary Ward), the Cambridge Rehabilitation Unit, and the Princess of Wales Rehabilitation Unit in Ely. We also assist with discharges from care homes following health‑led discharges. In addition, our team provides ongoing support after discharge by carrying out six‑week reviews to ensure individuals’ needs continue to be met in the community.
Transfer of Care Teams work within the following hours:
Monday to Thursday: 9:00am – 5:00pm
Friday: 9:00am – 4:30pm
Our team works in a hybrid model, combining time at the office with the flexibility of working from home.
We are committed to supporting flexible working opportunities and staff can also benefit from salary enhancements depending on their chosen working pattern:
- 7% enhancement – for working 1 weekend day per month and 1 Bank Holiday per year
- 8% enhancement – for working 1 weekend day per month and 2 Bank Holidays per year
- 10% enhancement – for working 1 weekend day per month and 3 Bank Holidays per year
- 15% enhancement – for working 2 weekend days per month and 3 Bank Holidays per year
In addition, staff who work one or two weekend days each month will receive a day(s) off during the week in recognition of their contribution. Weekend working arrangements are subject to review in line with local agreements.

What will you be doing?
You will join a group of supportive Senior Social Workers and be part of the management team.
Whilst this can be a fast-paced role, we are a highly supportive team offering excellent guidance and help from both colleagues and managers, alongside opportunities to develop your knowledge and skills.
Key responsibilities of the Senior Social Worker post include:
- Managing the practice and performance of the team to ensure timely and safe discharges from the hospital setting.
- Supporting the day-to-day practice of the Team.
- Quality assuring Care Act assessments, care and support plans, overseeing s.42 Safeguarding enquiries and completing audits.
- Ensuring accurate record keeping.
- Being part of a management team, managing and nurturing a team of professionals (Social Workers and Adult Support Coordinators) so that they can operate innovatively and creatively to meet the needs of Cambridgeshire residents.
- Building positive relationships with key service partners and making positive contribution to delivering good outcomes for people who have care and support needs.
- Effective management of resources (including financial) and assurance of quality to achieve the best outcomes for those being discharged from hospital.
- You will take a lead on highly complex cases, working creatively in partnership with others to manage risk. You will also provide leadership, through managing essential functions of the team including complex safeguarding cases.
Supporting your supervisees as part of ‘Our Conversations’ to ensure their wellbeing and professional development.
About you
You must be a qualified social worker with at least 2 years’ post ASYE experience and ideally have experience managing people.
We would particularly welcome applicants who already hold, or are working towards, a Practice Educator, Best Interests Assessor, or Approved Mental Health Professional qualification.
You will have worked with a complex caseload and have excellent knowledge of current legislation which will include the Care Act 2014, Mental Capacity Act 2005 and the associated statutory guidance.
You will have proven knowledge and skills for Safeguarding Adults.
You will be a person-centred practitioner with excellent communication skills and have the ability to work within a multidisciplinary setting.
You will be imaginative, dynamic and an excellent communicator with the ability to influence the workings of other services.
This is a fast-paced environment, and the successful candidate should be able to work with competing demands for services.
You must:
· Be a qualified Social Worker with a Social Work England registration
· Have the ability to work from a variety of locations and travel countywide independently
