The South Transfer of Care Team are looking for a full time Senior Social Worker, working predominantly from Cambridge University Hospital (Addenbrookes), based at 28 Long Road Cambridge.

You will join a group of Senior Social Workers and be part of the management team.

The Team provide support to adults being discharged from the acute hospital and rehabilitation settings.

The Transfer of Care Teams operate within specific office hours, but that does not mean we will not consider flexible working opportunities. You will work 1 day of one weekend per month, plus 3 bank holidays throughout the year, on these weeks you will still work 5 days (full-time) and have a day off during the week.  In return we will offer you a 7% enhancement on your salary.

Interview will be held virtually on Teams the week commencing 22/05/23.

There will be a short written exercise as part of the interview but we’ll give you all of the relevant information ahead of time.

What will you be doing?

Key responsibilities of the Senior Social Worker post include:

  • Managing the practice and performance of the team to ensure timely and safe discharges from the hospital setting.
  • Supporting the day-to-day practice of the Team.  Quality assuring Care Act assessments, care and support plans and completing audits.
  • Ensuring accurate record keeping.
  • Being part of a management team, managing and nurturing a team of professionals (Social Workers and Adult Support Coordinators) so that they can operate innovatively and creatively to meet the needs of Cambridgeshire citizens.
  • Building positive relationships with key partners and making positive contribution to delivering good outcomes for people who have care and support needs.
  • Effective management of resources (including financial) and assurance of quality to achieve the best outcomes for those being discharged from hospital.
  • Managing complex safeguarding cases and or serious concerns processes.
  • Supporting your supervisees as part of ‘Our Conversations’ to ensure their wellbeing and professional development.

About you

You must be a qualified social worker with at least 2 years’ experience and ideally have experience managing people.  You will have worked with a complex caseload and have excellent knowledge of current legislation which will include the Care Act 2014, Mental Capacity Act and the associated statutory guidance.  You will have proven knowledge and skills for Safeguarding Adults.   

You will be a person-centred practitioner with excellent communication skills and have the ability to work within a multidisciplinary setting. You will be imaginative, dynamic and an excellent communicator with the ability to influence the workings of other services.

This is a fast-paced environment which will requires a high level of personal resilience and the successful candidate should be able to work with competing demands for services.
