About the role
The Transfer of Care Team is seeking a bright, talented, and dynamic individual to fill the role of Service Manager. We are committed to high quality service delivery and development and improving the patient's experience, reducing length of stay and ensuring patients are discharged in a safe and timely manner. You should have previous experience in this area, which you are able to clearly demonstrate.
The role will work across 3 Acute Hospitals Addenbrookes (Cambridge), Hinchingbrooke (Huntingdon) and Peterborough City Hospital. We also support 5 Inpatient Rehabilitation centres across Cambridgeshire and Peterborough. The role is hybrid with a work base of either Hinchingbrooke Hospital (Huntingdon) or 28 Long Road, Cambridge (next to Addenbrookes)

What will you be doing?
Key area of responsibility will be the operational management of day-to-day hospital discharge and supporting the 5 Team Managers and Head of Service across sites in the achievement and maintenance of discharge performance and targets.
You will report to the Head of Service and be instrumental in the planning and development of systems and processes to support our residents being discharged from hospital.
The environment is dynamic, fast paced and challenging and you will require a high attention to detail as well as being able to work under pressure. In return you would be part of a fantastic team with a strong support structure
About you
You will be a qualified Social Worker registered with Social Work England.
Personal resilience, drive, focus and the ability to work to tight deadlines are essential requirements. The role would ideally suit someone with experience of discharge processes, with knowledge of sector pressures and desirable experience in operational management.
You will need to have strong inter-personal skills and be able to influence and motivate others and to support people through service improvement and transformational change.
You will be able to work as part of a team as well as demonstrating the ability to be able to work independently, with some previous management experience.
Being able to think innovatively and communicate effectively with a wide range of departments, teams and agencies is essential.
Fully proficient at using IT systems, with a good working knowledge of Microsoft Windows and associated office packages and the ability to use these packages to manipulate data and produce reports.
