Social Work Team Manager - Continuing Healthcare

About the role

Are you an experienced Team Manager, or interested in the opportunity to progress, with a background in adult social care?

An exciting opportunity for a permanent, full time, Team Manager in our adult Continuing Healthcare Team is available.  

You will be a confident leader who will hold the key responsibility for managing the delivery of high-quality professional practice in line with legislative frameworks, policies and standards, ensuring that statutory, national, and local performance targets are met.

A working knowledge and application of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2022) is essential.

The Continuing Healthcare Team are a county-wide team based in Huntingdon. We work in partnership with Cambridgeshire and Peterborough Integrated Care Board (ICB) to determine individuals' eligibility for NHS Continuing Healthcare through joint completion of the NHS Continuing Healthcare process.

The team also complete Care Act 2014 Assessments, Reviews, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) identifying individuals' needs and supporting the multidisciplinary team's recommendation on eligibility for NHS Continuing Healthcare. Where individuals are not eligible for NHS Continuing Healthcare but are eligible for care and support from Cambridgeshire County Council in line with the Care Act 2014, you will implement this.

In addition, the role may involve supporting Cambridgeshire County Council’s Adult Social Care workforce to improve their confidence, skills, and knowledge in relation to NHS Continuing Healthcare. You will provide training, coaching, mentoring and advice to staff in relation to the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2022).

Appointment to this post will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service. What will you be doing?

The role is varied on a day-to-day basis and allows for a high level of multi-agency working, creativity and autonomy. You will be part of an established group of team managers working across our adult social care services. You will manage a team of senior social workers, social workers and adult support coordinators, providing robust leadership, guidance and good quality support. 

You should be able to professionally utilise a strengths-based approach, and be able to make evidence-based decisions to promote the safety of others.

The service is busy, lively and interesting and would ideally suit a manager who is proactive and adaptable in leading a team, and who has a passion for ensuring that the adults receiving care and support in our area receive a positive experience when having their needs met.

About you

You must be a registered Social Worker with experience of managing people and performance. You will also have some experience of leading and managing change. You will be able to influence the workings of other services, and you will be a skilled communicator who can work with competing demands for services. 

Experience of working with adults with care and support needs and carers is essential along with a good understanding of the underpinning legislative frameworks which guide practice, including The Care Act 2014, Mental Capacity Act 2005, and Continuing Health Care guidance.

You will be able to travel independently between locations within Cambridgeshire, including to locations which are not on public transport routes.
