Social Worker - Care Home Support Team

About the role

An exciting opportunity has arisen for a full time Social Worker in the Care Home Support Team. The role is offered either as a 12 month secondment or on a 12 month fixed term contract.

The Care Home Support Team offers a fresh approach to supporting Care Homes, Nursing Homes and Supported Living settings across  Cambridgeshire.  We work alongside Care Home Managers, empowering them to find solutions to complex issues and coaching them through change management. The team works with Home Managers to understand the actions that are required, implement the necessary changes and help to embed a positive culture amongst staff within the home. The support is flexible, intensive and preventative. We work in partnership with the provider, Contracts team, CCG and other agencies to ensure effective and sustained change and delivery of high quality care to the residents.

What will you be doing?

You will work in a variety of ways with homes including face to face work, offering virtual support, and joint working with our colleagues from other quality practice teams (such as Contract monitoring, the Quality ICB team and the Medicines Optimising Team). You will  provide practical support with areas such as:

  • Care planning
  • Risk assessments
  • Mental Capacity Assessments
  • Deprivation of Liberty Safeguards
  • Activities
  • Technology Enabled Care
  • Safeguarding responses
  • Working with families

The support the team offer is in the form of mentoring support and completing assessments with carers and managers to ensure that they have the practical skills alongside the formal knowledge.

About you

We're looking for a confident, dynamic individual who enjoys building relationships, mentoring and exploring possibilities.  We welcome applications from those who may not have direct experience but can demonstrate transferable skills over experience in a similar role.

You must:

  • Be a qualified Social Worker with a Social Work England registration
  • Have the ability to work from a variety of locations and travel countywide independently
